Manage the communication that patients and referring doctors receive when a patient scores below an 8 for oral hygiene on a timeline post

• If a patient has a Referring Doctor selected on the new patient form both the Referring Doctor and Patient will receive Email Body - Dentist
• If a patient does not have a Referring Doctor selected they will receive Email Body - Patient
